Re: Home Care Services

Dear Mr. Cline:

Renown Health Home Care has spent some time researching your request to identify a mechanism to get your caregivers certified to provide personal care for you.
We have recommended the following options to you previously; these are:

· Schedule an appointment with your Primary Care Physician as recommended by Medicaid. Your Primary Care Physician is aware of your need and willing to accommodate .
· Work with contracted Medicaid services, which will provide personal care attendants and certify their competence to care for clients, as well as perform the necessary background checks.

As you know, Renown Health Home Care provides services to clients who have demonstrated a skilled nursing need. Unfortunately, as we have discussed, you do not meet our admission criteria and we do not provide the services you are requesting. At this time our program does not provide the services you are requesting, nor are there any plans to include this in our service line.

Sincerely,
Mike-Klepin-signature
